All cognitive mathematics blocks for the grade 12 mathematics DBA are 30 minutes long. Forms 256–259 are accommodation forms. An accommodation form contains blocks with all items or a subset of items from non-accommodation blocks (e.g., a bilingual form has a one-to-one correspondence to a non-accommodation form, but a high-contrast form may not present certain items). While form 256 (containing blocks M9 and M10, modified for accessibility) and form 259 (containing blocks M11 and M5, modified for accessibility) were offered for the 2019 grade 12 mathematics assessment, no students required them. Therefore, no students were assessed using these blocks. Only test forms for operational assessments are included in this table.
