During the preassessment visit, field staff provide school coordinators with school questionnaires and teacher questionnaires. In addition, if there are any newly identified students with disabilities or English language learners, field staff provide questionnaires for these students' teachers. Field staff request that the school coordinator return completed questionnaires on assessment day.
If field staff learn on assessment day that there are questionnaires that have not been completed, they ask the school coordinator to have school staff complete and return questionnaires before field staff depart. If this is not feasible, field staff provide the school coordinator with a postage-paid envelope in which completed questionnaires can be returned to the NAEP data collection staff.
