Average Assessed Value Per Acre for this Type of Property - Obtained by multiplying Average Value Per Acre for This Type of Property by the Adjustment Factor Used to Convert Market Value to Assessed Value.

Check if the Applicant's Boundaries have Changed Since the Last Filing for Impact Aid Funding - An indication of whether the applicant's boundaries have changed since the last filing for Impact Aid Funding, including through consolidation with one or more other LEAs.


Check if the Number of Acres of Any Eligible 8002 Property Have Changed Since the Last Filing for Impact Aid Funding - An indication of whether the number of acres of any eligible 8002 property have changed since the last filing for Impact Aid Funding.

Estimated Assessed Value for Portion of Federal Property Based on This Type of Adjacent Property - Obtained by multiplying Number of Eligible Acres times Proportions of Property Adjacent to Federal Property times Taxable Percentage times Average Assessed Value per Acre for This Type of Property.


Fax - The fax number of the person on the LEA staff who is most knowledgeable about the application.


Federal Agency - The name of the Federal Agency responsible for administering the Federal program from which revenues were received during the previous fiscal year from activities associated with section 8002-eligible Federal property.


Federal Program - The name of the Federal program from which revenues were received during the previous fiscal year from activities associated with section 8002-eligible Federal property.


Funds Received During Fiscal Year - The amount of revenues received from other Federal programs during the previous fiscal year from activities associated with section 8002-eligible Federal property.


Impact Aid Number - The number assigned by the Impact Aid Program to the applicant.


Name of Authorized Representative - The name of the LEA's authorized representative who can commit the applicant to the assurances contained in the application, including the assurance that the applicant meets all statutory, regulatory, and administrative requirements.


Name of Certifying Official - The name of the local official responsible for assessing the value of real property for levying real property taxes in the school district.


Name of Jurisdiction - The name of jurisdiction (e.g., county name) in which the Section 9002-Eligible property is located.


Name of Section 8002-Eligible Federal Property - The name of the real property to which the Federal Government acquired ownership on or after 1939.


Name of Taxing Jurisdiction - The name of the taxing jurisdiction (e.g., school district name, township name, city name, county name, etc.).


Number of Eligible Acres - The number of eligible acres in the real property to which the Federal Government acquired ownership on or after 1939.


Original Application - An indication that the LEA has applied for Impact Aid in previous years. (Will be pre-populated.)


Phone - The telephone number of the person on the LEA staff who is most knowledgeable about the application.


PR/Award Number - The PR/Award Number for the applicant.


Proportion of Property Adjacent to Federal Property -


School Year - The school year for which the Impact Aid Funding is requested.


Signature - The signature of the LEA's authorized representative.


Signature of Tax Assessment Official - The signature of the local official responsible for assessing the value of real property for levying real property taxes in the school district.


State & Zip - The state and zip code of the address of the applicant.


Street/P.O. Box - The street or post office box address of the applicant.


Tax Rate for Current Operating Expenditures for This Type of Property for Fiscal Year - The local real tax property tax levy, in mills or dollars, that was used to raise funds for current operating expenditures for the local educational agency (LEA) for the fiscal year (e.g., 2.5 mills per $1.00 or $2.50 per $1000).


Taxable Percentage of Property Adjacent to Federal Property -


Telephone Number of Certifying Official - The telephone number of the local official responsible for assessing the value of real property for levying real property taxes in the school district.


Title - The title of the person on the LEA staff who is most knowledgeable about the application.


Title of Certifying Official - The title of the local official responsible for assessing the value of real property for levying real property taxes in the school district.


Title of the Authorized Representative - The title of the LEA's authorized representative.


Total Acres of This Type of Property - The total number of acres for this type of taxable real property in the local educational agency (LEA).


Total Assessed Value of This Type of Property - The total assessed value for tax purposes for each type of taxable property in the local educational agency (LEA).


Total Estimated Assessed Value for Portion of Federal Property Based on This Type of Adjacent Property - Computed for all instances of Section 8002-Eligible Federal Property. (Changes of more than 10 percent or more from last year's submission are to be explained.)
